Forms must be submitted prior to school attendance.
Required for all* families every year:
- Non-Public Transportation Reimbursement Qualifying residents may submit each year by law to receive reimbursement pay for travel expenses. The school is required to have a copy on file for participating families. *Minneapolis residents do not qualify for this reimbursement and do not need to submit the form.
Required for new families, kindergartners, and 7th graders (if we have this on file, we do not need another one):

- Medication Authorization Must be completed each year if any prescription or OTC medication is to be administered at school. Medication must be provided by the family.
